 The 4-th Microsurgical Department

The IV-th microsurgical department of the Ufa Eye Research Institute is a multi-field department organized in 1982.

Each year about 2,000 patients with different ophthalmopathology get surgical and conservative treatment in the department.

Doctors carry out:

• Vitreoretinal surgery of retinal detachment

• vitreoretinal surgery in proliferative diabetic retinopathy

• vitreoretinal surgery in IOL and lens dislocation into the vitreous

• vitreoretinal surgery of hemophthalmia

• Seamless microinvasive vitreoretinal surgery (23, 25G)

• Extrascleral retinal detachment surgery

• Removal of foreign bodies of any location, regardless of their magnetic properties

• Reconstructive surgery in trauma

• Intravitreal administration of drugs (triamcinolone, Lucentis)

• Surgical treatment of cataract: phacoemulsification, extracapsular and intracapsular extraction with implantation of intraocular lenses of leading foreign and domestic manufacturers.

• Surgical treatment of glaucoma, including combined treatment of cataract and glaucoma, fistulizing operation, operations with the use of drains, cyclodestructive operations.

• Surgical treatment of benign and malignant eye tumors and its appendages.

Currently, there are 4 surgeons in the Department including 2 doctors of the highest qualification category and 2 MDs. Besides, since 2006 there are 4 ophthalmologists of  vitreoretinal department performing surgeries in the 4 Department. Specialists underwent vitreoretinal training at ISTC "Eye Microsurgery", Moscow Helmholtz Research Institute of Eye Diseases, Germany, and annually take part in scientific conferences, congresses in Russia and abroad.

Department conducts surgical and conservative treatment of patients with different ophthalmopathology. However, priority branch now is retinal and vitreous pathology.  The great experience is gained in surgical treatment of one of the most severe eye diseases - retinal detachment. However, this type of pathology is much rarer than, for example, cataract or glaucoma, it requires special attention because of its severity, difficulty of treatment and it more often causes disability due to the late ambulation. In the detached retina an irreversible degenerative processes occurs quickly even with a favorable outcome of the surgery. Department developed and implemented proprietary methods of scleroplastic operations.

The operating room of the Department is equipped with the most modern equipment of leading foreign companies (operating microscopes, vitreous cutter, phacoemulsificators, the setting for endolasercoagulation, etc).

We provide a treatment for retinal detachment of various etiologies, proliferative diabetic retinopathy, hemophthalmia, dislocation of the lens and IOL in the vitreous body,  removal of magnetic and non-magnetic foreign bodies from the posterior eye segment. Depending on the extent of detachment sectoral or circular indentation of the sclera is performed including ballooning. The goal of surgery is to block the retinal breaks, to ease tractions in the vitreous body. In cases of traction retinal detachment with severe proliferative changes of vitreous vitrectomy is performed. In such situations  a temporary tamponade with air-gas mixture or vitreous substitutes is used. The final step is laser retinal coagulation.

When PDR is performed vitrectomy and removal of preretinal membranes is performed. The operation is done by accessing at 3-points through the flat part of the ciliary body, with the use of irrigation systems, endovitreal light guide. Then a total vitrectomy is carried out with the removal of membrane, during the operation PFOC or silicone oil and endolaser coagulation are applied.

The aim of vitrectomy with hemophthalmia is maximum removal of the changed vitreous and streamed blood. Maximum removal of the vitreous is the prevention of the toxic effect of blood on the microstructure of internal membranes of the eye and the lens, it prevents the formation of synechia and tractional retinal detachment.

At dislocation of the lens into the vitreous vitrectomy is performed to remove the tractional pressure and lensectomy with PFOC. At IOL dislocation vitrectomy, removal or reposition of IOL, reimplantation with different types of fixation of intraocular lens is performed.

When there are foreign bodies in the vitreous, posterior segment vitrectomy, removal of foreign body is performed, if necessary endodiathermocoagulation and endolasercoagulation of retina and PFOC tamponade is performed.

The 4 microsurgical department provides conservative treatment of vascular diseases (retinal vascular occlusion, diabetic damage), hereditary and inflammatory diseases of the retina and vascular tract, diseases of the optic nerve, degenerative diseases of the retina.

High qualification of the surgeons of Department in combination with high-tech equipment allows to treat and operate patients at the highest level.
